The Patient Centered Medical Home

At Western Valley Family Practice our goal is to provide quality, caring, patient-centered healthcare. We provide team based care using the skills of different health care providers to maximize the health of our patients. We want our patients to feel they are being viewed by the doctors and staff, as unique, whole people, with individual needs. We will attempt to understand each person’s needs by listening carefully then doing our utmost to meet them.

At Western Valley Family Practice we have adopted The Patient Centered Medical Home philosophy of health care delivery. In practice, this means a number of things.

  • Our patients have an ongoing relationship with their personal provider who is trained to provide first contact, continuous and comprehensive care.
  • This personal provider leads a team of individuals at our practice who take responsibility for the ongoing care of our patients.
  • This personal provider is responsible for providing for all of the patient’s healthcare needs or taking responsibility for arranging care with other qualified professionals. This includes care for all stages of life; acute care; chronic care; preventive healthcare; and end-of-life care.
  • Our patient’s health care is coordinated across all elements of the healthcare system including hospital, home health, nursing home, assisted living facilities. This care is enhanced by use of patient registries, information technology and a health information exchange to assure that patients get the indicated care when and where they need and want it in an appropriate manner.
  • Our providers use the best available evidence to deliver appropriate treatment recommendations. We participate in an ongoing program of continuous quality improvement.
  • We provide our patients with enhanced access to care though same-day appointments, telephone and secure e-mail communications. We are available 24 hours a day, 7 days a week to provide care and advice to our patients.

Patient and Family Advisory Council

Western Valley Family Practice has introduced a Patient Advisory Council!

The Patient Advisory Council will operate as the voice of the patients who are currently obtaining care from Western Valley Family Practice (WVFP).

The Patient and Family Advisory Council (PFAC) will have an entrusted and enthusiastic role in helping as Western Valley improves its policies and procedures that will directly affect our patients and their families experience in our practice in a positive way.

The PFAC meets twice a year to allow YOU the patients to be able to share constructive criticisms that will help WVFP to become a better place, to aid in encouraging and confident and consistently positive relationship between WVFP its patients, their families, and to contribute to any planning, implementations, and evaluation of any quality improvement projects.

The PFAC consists of 12 members form our community with diverse backgrounds and having a combination of Primary Care Physicians within our practice. Check back for more information regarding our PFAC and some updates from the members!

Ear Piercing

Western Valley Family Practice now offers Ear Piercing!

Dr. McClellan has started the service of ear piercing for the practice. The cost is $35 and the service is available to anyone, whether patient or not, by appointment. The $35 is due at time of service. Please call our office for more details.

Walk-In Visits

We offer Walk-In Visits for current WVFP Patients.

Walk-In Visits include Blood Pressure Checks, Weight Checks, Flu Shots (when available), and injections when approved by your Primary Care Physician (PCP).

Walk-In Visits are welcome Monday – Friday from 8:30 am – 4:45 pm, with the exception of the lunch hour (12pm – 2pm) to ensure a Doctor is present. Please contact our office for any questions regarding our walk-in visits, other situations may apply.

Lab Draw Stations

Our Fruita Lab Draw Station is open Monday – Thursday from 8:00 am – 5:00 pm, closing for lunch from 12:00 pm – 12:30 pm, and Friday’s from 8:00 am – 12:00 pm.

Our Redlands Lab Draw Station is open Monday – Thursday from 8:00 am – 4:00 pm and Friday from 8:00 am – 3:00 pm, closing over the lunch hour from 12:00 pm – 1:00 pm.
